Cost includes time required for treatment plan & referrals. After booking, you will an email with a link to our New Patient Form. This will need to be filled out before your consultation for both patients. Please check your junk folder if you do not see it.
Cancellation Policy
Cancellations within 48 hours of scheduled appointment will attract a 50% consult fee.
