Why Real Results Take Time: The 6-Month Programs at RH Reproductive Health

Real lasting change takes time. That's the premise behind everything we do at RH Reproductive Health, and it's the reason we've built these programs around a 6-month framework rather than a quick fix.

The medical model is built for speed. You come in with a symptom, you get a prescription, you go home. And for some things, that's exactly what's needed. But for hormonal health, fertility and gut health, a quick solution rarely promotes long-term health.

Quick fixes produce quick results. The problem is that those results don't last because they never addressed what was driving the problem in the first place. A short course of supplements without proper investigation. An elimination diet that gives partial relief but never finds the root cause. A GP panel that comes back "normal" when normal and optimal are not the same thing.

If you're ready for deep, personalised support and want to explore whether one of these programs is the right fit for you, please book a complimentary 15-minute call and complete the short application form.

This helps me understand your goals and what kind of support you are looking for, so I can best guide you before we speak.

That's the gap I built these programs to fill. Three structured, 6-month programs at RH Reproductive Health: the Male Vitality Program, the Perimenopause Transition Program, and the Gut Reset Program. Each one starts with proper investigation, builds a targeted treatment plan around your specific findings, and gives you the time and structure to see real change.

In this post, I want to explain how these programs work, what makes them different, and why proper diagnostic testing is the foundation of all of it.

Why Structured Programs Work When Quick Fixes Don't

There's a reason so many people feel like they're not making any progress with their health. The generic wellness industry is excellent at creating the feeling of progress: the new supplement, the elimination diet, the morning routine. But none of that addresses the underlying cause of the problem.

What I see in practice, again and again, is that the people who actually get better are the ones who:

1.     Identify the specific drivers of their condition through proper investigation

2.    Follow an individualised plan that is targeted to those findings

3.    Stay consistent long enough for change to occur

The third point is where most people fall short. Not because they're not motivated, but because nothing is holding the plan together. No structure. No clear milestones. No accountability between appointments.

That's what a structured program provides. It creates a container: a clear start, a defined duration, regular check-ins, everything you need already in place. Your energy goes into following the plan, not managing the logistics.

The 6-Month Window Matters

Many of the changes we're working towards take time. Hormonal shifts, body composition, gut microbiome remodelling: these processes don't respond to short interventions. Hormonal patterns can take 2 to 3 cycles to stabilise. Gut microbiome shifts are gradual and require sustained consistency to hold.

A six-month program isn't arbitrary. It's the minimum meaningful window for many of these processes to show real change.

What's Included in All Three Programs

Before I walk through each program individually, here's what all three have in common.

Initial and follow-up consultations are built in. We move from investigation to treatment plan in a structured way, and you have five check-in points throughout the program to reassess, refine, and adjust.

Targeted supplementation is included for the duration of the program. The supplements I've selected for each program are clinically specific, dosed correctly, and ready to go. If additional supplementation becomes clinically indicated as your program progresses, I'll discuss that with you. But the core protocol is already covered.

Pathology testing is integrated into each program, specific to the clinical focus of that program. Without testing, we're guessing. With testing, we have a map.

Who it's for: Males who want to optimise their hormonal health, whether that's for fertility, energy, body composition, sexual health, or simply feeling like themselves again.

Male hormonal health is one of the most under-investigated areas of medicine. Most patients are told their testosterone is "fine" based on a single marker, total testosterone, without any context about what that means for their individual physiology. No one asks about free testosterone, SHBG, LH, FSH, or the dozens of other markers that actually tell the full story.

That's exactly what this program addresses.

A Hormone Check: 47 Markers, One Comprehensive Picture

This program includes a comprehensive blood panel of 47 markers covering hormonal function, cardiovascular risk, and nutritional status.

This isn't a standard GP panel. This is the kind of deep picture that lets us identify:

·       Where your testosterone levels actually sit, and whether "normal" is normal for you

·       How your body is producing and converting hormones (LH, FSH, SHBG, estradiol)

·       Your thyroid function in full, not just TSH in isolation

·       Your adrenal function with cortisol and DHEA

·       Key metabolic markers, including blood glucose, insulin, and lipid panel

·       Inflammatory markers

·       Nutritional deficiencies that affect energy, mood, and hormonal health

Why does this matter? Because the solution to low energy isn't always the same. For one person, it might be low testosterone. For another, it's iron deficiency. For another, it's thyroid dysfunction. For another, it's cortisol dysregulation. Without testing, you're guessing. With 47 markers in hand, we can identify exactly what's driving your picture and build a targeted plan around that.

Who it's for: Females navigating perimenopause who want to understand what's happening in their body, manage their symptoms effectively, and move through this transition with clarity rather than confusion.

Perimenopause is one of the most hormonally complex periods of a female's life, and one of the most underdiscussed. Oestrogen levels fluctuate wildly before they decline. Progesterone drops. FSH rises. Sleep becomes disrupted. Mood shifts. Weight distribution changes. Many are told, "This is just part of getting older", and sent on their way.

It doesn't have to feel that way.

A Comprehensive Hormone Test: A Snapshot That Changes Everything

The Perimenopause Transition Program includes a thorough blood panel of 15 key markers that gives us a full picture of where you actually are in your hormonal transition.

The panel covers:

·       Reproductive hormones: Oestradiol, progesterone, testosterone, DHEA-S

·       Adrenal hormones: DHEA-S and cortisol, because your adrenals matter enormously during perimenopause

·       Thyroid function: TSH, Free T4, Free T3, and thyroid antibodies. Thyroid dysfunction is extremely common in this life stage and is frequently missed

·       Pituitary signalling: FSH and LH, which tell us where you are in the menopausal transition

·       Prolactin and SHBG: which affect both hormonal balance and how available your sex hormones actually are

Why this panel? Because symptoms alone don't tell the full story. Hot flushes, brain fog, mood changes, and irregular cycles can be driven by half a dozen different hormonal patterns. Without testing, you're managing symptoms. With testing, you're addressing causes.

Who it's for: People with chronic digestive symptoms: bloating, irregular bowel habits, food intolerances, abdominal discomfort. People who want to understand what's actually happening in their gut, not just manage symptoms indefinitely.

Gut health is one of the most talked-about topics in wellness, and one of the most poorly investigated. Most people I see who have gut issues have been through the elimination diet cycle. They've removed gluten, then dairy, then FODMAPs, then something else. While they've had some relief, they've never had a real answer.

That changes with this program.

Microba: Shotgun Metagenomics Testing

The Gut Reset Program includes Microba gut microbiome testing: one of the most advanced gut analysis tools available in Australia.

Microba uses shotgun metagenomics, which means it sequences the full DNA of every organism in your gut sample, bacteria, fungi, archaea, and parasites, rather than just looking for specific pre-selected targets. This gives us a complete map of your microbiome: what's there, what's missing, and how it compares to a healthy reference population.

The clinical information this provides is significant:

·       Microbial diversity: Low diversity is consistently associated with poor metabolic health, immune dysregulation, and mood disturbance

·       Specific bacterial species: Some species are protective; some drive inflammation; many have direct effects on gut motility, gas production, and intestinal permeability

·       Short-chain fatty acid production: The metabolites your bacteria produce affect everything from gut lining integrity to blood sugar regulation to brain function

·       Potential pathogens: Organisms that may be contributing to your symptoms and that can be specifically targeted

The Microba report comes with a clinical interpretation. We don't just give you a list of bacteria and send you home. We use the findings to build a targeted protocol, including specific dietary strategies, prebiotics, and probiotics chosen for your individual microbiome picture.

Why Work With Me

I'm Raul Pastrana, naturopath at RH Reproductive Health in Melbourne.

I built these programs around a single principle: investigate properly first, then treat specifically.

Every person who works with me gets a thorough case history, a targeted testing protocol, and a treatment plan built around their specific findings. Not a template.

Because we are all different, what drives low testosterone in one person is different from what drives it in another. What's destabilising one person's perimenopausal transition is different from what's destabilising another's. The protocol that resolves one person's bloating will do nothing for the next person's.

The programs on this page represent the clinical framework I use every day, packaged in a way that makes it accessible, structured, and consistent. They exist because real change requires real investigation, and real investigation takes time.

I am also the author of The Male Factor, a clinical reference on male reproductive health, and I work with individuals and couples across all stages of the fertility journey, from preconception optimisation through IVF preparation and beyond. My approach is inclusive, evidence-based, and honest about what we know and what we don't.
